Community Support workers provide advocacy, ensure continuity of care between multiple service providers, support persons served during times of crisis, and provide training for independent living skills.
A comprehensive assessment is completed and used to develop an individualized service plan, integrating the strengths and abilities of the individual with the needs and preferences identified.
Assistance in acquiring needed resources, such as SSDI and other entitlements, offers the individual greater community and social integration.
Community Support provides services to individuals living in South Central Nebraska with a primary diagnosis of major mental illness and/or substance dependence.
The role of Community Support staff varies, depending upon the needs and choices of the person served.
The service is generally provided in the community and/or at the individual's residence as appropriate.
Average length of stay in the program varies greatly depending upon the on-going needs of the individual. Regular treatment team meetings are scheduled in order to facilitate continuity of care, and to provide a forum for the individual to discuss progress, needs, and direction for service delivery.
Eligibility for participation in Community Support services includes some or all of the following criteria as defined by the State of Nebraska:
- Age 19 or older
- At risk of hospitalization or severely dysfunctional living
- Demonstrate functional impairment which substantially interferes with two or more major life areas
- A primary diagnosis of schizophrenia, major affective disorder, or other major mental illness
- A primary diagnosis of alcohol and/or substance abuse/dependence
